Clinical privacy notice
Health care providers are generally required to make a Notice of Privacy Practices available to clients. The notice explains how protected health information may be used and disclosed, client privacy rights, and the practice's responsibilities for protecting health information.
Clients may receive the complete current notice through the secure client portal or practice documents provided before services begin. This online version should be kept aligned with the practice's official notice and any applicable Washington or federal requirements.
Your privacy rights may include
- Requesting access to your health record
- Requesting corrections to health information
- Requesting confidential communications
- Requesting limits on certain uses or disclosures
- Receiving a copy of the current notice
- Filing a privacy complaint without retaliation
Uses and disclosures
Protected health information may be used or disclosed for treatment, payment, health care operations, and other situations allowed or required by law. Some uses or disclosures may require written authorization.

Appointment requests, intake paperwork, and client communication are handled through the secure client portal. Please do not include sensitive clinical information in standard email or website messages.
